New Patients

First Visit

The initial visit includes consultation of goals, health history, movement assessment, examination, diagnosis, and treatment. It usually lasts 45 minutes. Subsequent treatments last 15 minutes. Please bring all necessary files and paperwork filled out. You can find forms here. If you are unable to complete paperwork, please arrive 15 minutes early in order to do so. If you have a lower extremity issue, please bring shorts. If you are female and have an upper extremity issue, please wear a tank top or shirt that can expose the problem area.

Payment

It is our policy to collect all fees at the time of service. Convenient methods of payment include: cash, check, or credit card.

New patient exams $145
Standard visits $75

**Referrals and Crossfit members get a 10% discount.
New patient exams $125
Standard visits $65

A $130 fee is added for each at-home request.

Insurance

We do accept most insurance coverage.

However, we are not in-network with any insurance companies.  We suggest that you check with your insurance company to find out if you have out-of-network chiropractic or physical therapy benefits.  If not, we can verify benefits for you.

It is our policy to collect all fees regardless of whether you have benefits or not.  You will be reimbursed based on what your insurance company covers.
